SECTION I - GENERAL INFORMATION
Length of Application
The application must stay within the three pages. However, you may adjust the length of section II based on your needs. Ex: If you are able to describe your project in less space than provided, you can shorten that section and utilize the available space in your objectives. Or, if you only have two objectives you can shorten the objective three and make your other two objectives a little longer. Again, the end result must be no more than three pages including the budget.
Communication Plan-Required
All applicants must also submit a brief (one-page) communication plan for their project. The communication plan needs to explain at least three ways you plan to communicate your project to the general public or to specific audiences. For example: press releases to newspapers and radio stations, public speaking engagements to local organizations, students sharing their project results with other students. Communication can and should occur through all phases of the project. If you complete the application online you will need to e-mail (info@rootpikewin.org) us the communication plan.
Project Name:
Designate a name for your project that briefly describes the content/intent.
Project Sponsor:
What is the name of the organization that will take responsibility for seeing the project through to completion or at whose request/suggestion this project is being undertaken (This organization should also be able to act as the fiscal agent for your project. WIN does not dispense funds to private individuals.)
Is the project sponsor a (please check one):
- 501 C (3): A designated non-profit organization
- School: Any academic institution (elementary, middle, high, college, university, etc.)
- Govt Agency: State or local government
- Other: Any organization that does not fall within the other three categories
Project Leaders Name and Organization:
Name of the person responsible for design and implementation of the project. This is the person who will be used as the contact person for any questions arising about the project. The organization they are affiliated with may be different than the sponsoring organization as long as there is an agreement between the two organizations on the support of this project.
